This zodiac man is found in an English farmer's almanack from the late 14th century.  The entire almanack is written on six sheets of vellum, each of which is folded up and kept in a leather pouch. 

Beside the zodiac man is a table that corresponds the months to the zodiac. Instead of writing the zodiac names within the table, the artist has drawn their symbols. The fish in a diagonal line, for example, each represent Pisces.
